West Community Connect makes it easy for health service providers to find mental health, addictions, community support and seniors services for their patients and clients. We work with people who live in the West Sub-Region of Toronto.

What We Do

 West Community Connect provides a single point of access to a range of community          supports and health services including:

  • A spectrum of mental health services from counselling and case management to intensive wrap-around services
  • Drop-in and group programs
  • Addictions services including withdrawal management and residential treatment
  • Personal support services
  • Meals on Wheels
  • Seniors day programs and respite care

To be eligible for service, your patient/client must live in one of the following postal codes: M6K, M6M, M6N, M6P, M6R, M6S, M8V, M8X, M8Y, M8Z, M9A, and M9N.

How It Works

To access our services, health service providers use the West Community Connect portal to help find a service, get an appointment, or contact someone to help coordinate multiple services for a client.

Provider selects service and books an appointment for client with a community provider directly or the Hub to coordinate services

Crisis and Immediate Supports

There are a number of immediate and crisis supports available in Toronto – some of these resources are listed below. In the event of an emergency, please dial 911 or visit your local emergency department.

 Assaulted Women’s Helpline – (416) 929-5200

The Assaulted Women’s Helpline is a free, anonymous and confidential 24-hour telephone and TTY crisis telephone line to all women in the province of Ontario who have experienced any form of abuse. The Helpline provide crisis counselling, safety planning, emotional support, information and referrals accessible 7 days a week, 365 days a year.

Crisis Outreach Services for Seniors (COSS) – (416) 929-5200

An on-call mobile crisis intervention and outreach service for older adults with addictions issues or mental illness. The COSS team provides direct in-person contact with clients within 24 to 48 hours in a community setting.

 Gerstein Crisis Line – (416) 929-5200

Community based crisis service for adults who are dealing with a serious mental health issue, concurrent or substance use issue and are currently in crisis.

The Warm Line

A confidential and anonymous service for adults living in the GTA and Peel Region in need of a friend with an empathetic ear. It is operated by peers who have experienced loneliness, isolation and anxiety.

 Big White Wall

At the heart of Big White Wall is its community of members, who support, help each other and share what’s troubling them in a safe and anonymous environment. The site is available 24 hours a day, 7 days a week, 365 days a year – no need to wait until morning, the next business day or for a doctors’ appointment. You can find support simply by logging on.

Distress Centres of Greater Toronto – (416) 408- HELP (4357)

Provides callers with emotional support and crisis intervention when they are socially isolated, marginalized, struggling with chronic mental health problems, in distress, attempting to navigate family violence or in need of emergency response or suicide prevention + intervention + postvention.

Toronto Withdrawal Management System – (416) 929-5200

Central Access is a primary point of entry into the Toronto Withdrawal Management Services system referral system for individuals 16 years of age and older (operated by St. Michael’s Hospital).
